PNIE stands for Psycho-Neuro-Immuno-Endocrine: the idea that the mind, nervous system, immune system, and hormones function as one interconnected system rather than four separate ones. This piece breaks down how each part communicates with the others, and why chronic and complex health conditions rarely make sense until you look at the whole system together.
What PNIE Stands For
Let’s talk about something really important — the PNIE system. It stands for: Psycho-Neuro-Immuno-Endocrine. Basically, it is how the mind, nervous system, immune system, and hormones all work together.
In medicine and healthcare, we often separate things into neat boxes. We look at the psychology on one side, neurology over here (that’s the structure and function of the nervous system), immunology (the immune system), and then the endocrine system, which is the hormone system. In truth, however, these systems are deeply connected. In recent years, there has been a growing body of research showing just how interconnected they are1 and honestly, understanding this is crucial if we are going to make sense of chronic and complex health issues.
Why I Stopped Treating Parts
The context from my own journey began over 25 years ago, when I started noticing patterns: why do so many people with back pain also have irritable bowel syndrome? Why do people who had tonsillitis as children often end up with throat problems, thyroid issues, or even frozen shoulders? These patterns weren’t random. The way medical professionals are trained is to look at each problem in isolation — as if the body is just a collection of unrelated parts.
I don’t treat parts, I treat whole people.
That approach completely changes how we understand health.
The Nervous System’s Role
To break this down: the nervous system does a lot more than just help a person think or move — it also controls the autonomic functions, like digestion, heartbeat, and what the internal organs are doing.
The Immune System’s Role
Then the immune system, which is like the body’s defence team. It is always scanning for threats, like infections or injuries, but it doesn’t work alone — it takes signals from the nervous system, which tells the immune system when to ramp things up, and vice versa. If the immune system senses a problem, it sends alerts back to the brain.
The Psychological Element
Then the psychological element comes in — events like trauma, stress, emotional overwhelm, and PTSD all have a massive impact on the nervous system. That includes areas like the amygdala (the brain’s alarm system), the midbrain, and even the vagus nerve — which plays a big role in calming the system down.
So, when the brain is getting signals that say, “We’re not safe! We’re not safe!” over and over again — even when there is no real danger — it starts a cascade. The immune system kicks into gear, sending out inflammatory cytokines (the chemicals involved in inflammation). Even the brain gets affected by this, with inflammation happening at the microscopic, cellular level.
It also activates T-cells, NK cells (natural killer cells), and a whole range of immune responses. Basically, the body is acting like it is under attack — sending all the troops out — when there might not even be a real threat. It’s exhausting.
The Hormone System’s Role
Let us now consider the role of hormones. When there is a threat response — for example, someone almost gets hit by a car — the adrenal glands release adrenaline. That is a good thing in the short term for prompting quick reactions. But if that stress keeps going, adrenaline release changes to cortisol. Cortisol is more of a slow-burn stress hormone, and it keeps the person on edge, ready for more threats. Over time, this has effects all over the body — including the thyroid, pituitary gland, and even sex hormones.
There is a link, for example, between oestrogen and migraine2 — so, if the nervous system is already dysregulated and oestrogen levels shift, like during a menstrual cycle, it can contribute to things like migraines or mood changes.
When the System Gets Stuck
This is all normal in a healthy system, but the problem starts when these systems get stuck in stress mode. Chronic stress and trauma can throw off the whole balance between psychology, nervous system, immune system, and hormones. This is why, when talking about chronic illness, it is not just about inflammation or hormones in isolation — they are working together, and when one goes off track, the others follow.
Consider: Crohn’s disease, pericarditis, inflammatory bowel disease, eczema, asthma — these might all be linked to a system that is stuck in a defence state, constantly being told “you’re under threat.”
For instance, mast cells are normally quiet until they get the signal to go and help with healing. But if they are constantly being triggered, they release things like histamine and other inflammatory chemicals3 — which leads to symptoms like itching, swelling, redness, and more. If this goes on for too long, the body starts attacking itself, which is the definition of an autoimmune disease. It is not that the body is ‘making a mistake’ — it is following the signals it is getting.
Why the Body “Attacks Itself”
“Why would the body attack itself?” It doesn’t make sense, but once we understand how the nervous system, mind, immune system, and hormones all talk to each other, it finally has some logical reasoning. This is why just treating symptoms — for example, using an anti-inflammatory for pain — doesn’t always get to the root of the issue.
As I often put it: if you’ve got a leaky roof, and a few tiles have slipped, fixing just one tile won’t stop the leak. Maybe the whole roof needs looking at. Maybe the foundation of the house is shifting, and that is causing the tiles to slip.
That is what I am really passionate about — understanding the root causes of health issues and how these systems relate to one another — because when we stop treating people as a collection of parts, and start seeing them as a whole system, that is when real healing can begin.
